1) Complete the LDT Intake Questionnaire (This is different from the application you already filled out)
By February 1, 2022
1) Notify Client Care if you would like to set up a 10-month Payment Plan, then follow instructions sent to you.
NOTE: This option includes 10% finance fee on your remaining balance.
2) OR by March 1, 2022, pay off your remaining balance if paying in full.
